The New Multi Agency Referral Form (MARF)

The LSCB is planning to change the Multi-agency Referral form (MARF) from Friday 18 September 2015. The new form has been designed by a multi-agency working group and introduced for several reasons.

  1. To simplify the form
  2. Reduce the amount of information required
  3. Utilise the Signs of Safety and Well-Being Framework

The new form is a Microsoft Word document that guides the referrer through the information that is required. The form has hints and tips as well as links to relevant information/guidance.

The process remains the same. When the practitioner has finalised their referral, they should save the form both as a word document and as a PDF. The PDF version is then emailed to Customer First at Suffolk County Council (email:

If the referral is for a child who is at significant risk, the referrer should contact Customer First by telephone on 0808 800 4005.

We plan to utilise this form for at least the next three months. We will then review the form with referrers, to identify how the form could be further improved.
